REMINDER: Activity Center Registration due 8/3

Dear JCS Community,

If you would like to use our After School Care Program, THE ACTIVITY CENTER in the 2026-2027 school year, please complete the registration form below.  We want to accept all families who need care but, in order to ensure appropriate staffing levels, this form must be completed by Monday, August 3.

2026-2027 Activity Center Registration Form: Please create a separate response for each child who will be attending. 

Important information:

JCS Activity Center is a part of our Expanded Learning Opportunities Program (ELOP).  

Hours: 2:15-5:30 Monday-Friday

August 19, 2026-June 9, 2027

The Activity Center is closed during school holiday breaks.

All school rules apply. 

Cost:

 $4.00 per hour

$0.40 per hour for students who qualify for reduced price lunch

Free for students who qualify for free lunch.

• ELIGIBILITY GUIDE and FREE AND REDUCED LUNCH APPLICATION

 for free/reduced price care.  An application MUST be completed in order for your student to qualify.
